Download, print and complete the required form with the necessary information. Kindly scan or take a photo of the completed form along with the proof of payment and email them to ctcadmissions@ctc.edu.jm. As an alternative, the document and proof of payment can be uploaded on the Forms Submission Portal (link available below).
